PLAYER REGISTRATION FORM
Winter League 2020/21
The following only applies if you have u18’s playing in teams as part of the colchester netball league safeguarding rules within the handbook and to ensure a duty of care to u18 players playing in the adult league. All teams with u18’s must adhere to the criteria below and if any advice is required please ask.
PARENTAL CONSENT FORM FOR U18’S
Being the parent or guardian of the participant, we need you to give permission for your child to play in the ADULT Colchester Netball League. Please ensure you have considered and understood the nature of such events and are satisfied to assume full and entire responsibility for your child’s own safety whilst engaged in any adult league games.
UMPIRE REGISTRATION FORM
Please could I ask that you complete the following form. We aren’t adding your details to the handbook or website as Facebook seems to be working well for communication for games but it’s useful for me as the officiating lead to have email/mobile details (should you wish) so that I can contact you with any umpire information or opportunities.
SUPPLEMENTARY PLAYER REGISTRATION FORM
Winter League 2020/21
Please complete this form for any new players you wish to register with your team during the season. (All details must be provided otherwise the player will NOT be registered.) The completed form should then be emailed to the Fixtures and Membership Secretary Kim Watson (firstname.lastname@example.org); alternatively, it can be deposited in the results post-box on the night.
INCIDENT OR ACCIDENT REPORT FORM
Guidelines for dealing with an Incident/Accident. Stay calm but act swiftly and observe the situation. Listen to what the injured person is saying. Alert the first aider who should take appropriate action for minor injuries. In the event of an injury requiring specialist treatment, call the emergency services. Deal with the rest of the group and ensure that they are adequately supervised.
SOCIAL MEDIA ACCEPTABLE USE AGREEMENT
